Use the practice’s secure SimplePractice portal or another office-approved secure method for sensitive information. The consultation link directs you to that portal. Do not submit diagnoses, treatment details, Social Security numbers, insurance identifiers, or medical records through ordinary website forms or email.

Appointments, fees, and insurance

Information on this site does not guarantee an appointment, insurance coverage, reimbursement, a particular fee, or a clinical result. Confirm availability, costs, coverage, and service details with the office. Clinical consent, payment, cancellation, and other treatment-related arrangements are addressed separately during intake.
