This page explains important privacy, billing, accessibility, and emergency information for clients and visitors of B & M Health Services, Inc. It does not replace individualized clinical advice or the documents provided during intake.

Notice of Privacy Practices

Your information. Your rights. Our responsibilities.

This notice describes how medical and mental health information about you may be used and disclosed and how you can obtain access to this information. Please review it carefully.

Your rights

You have the right to:

  • Obtain an electronic or paper copy of your health record and other health information we maintain about you. We generally provide a copy or summary within the time required by law and may charge a reasonable, cost-based fee.
  • Ask us to correct health information you believe is inaccurate or incomplete. We may deny a request in limited circumstances, but we will explain the reason in writing.
  • Request confidential communications, including asking us to contact you in a specific way or at a specific location. We will accommodate reasonable requests.
  • Ask us to limit what we use or disclose for treatment, payment, or health care operations. We are not always required to agree. If you pay for a service in full out of pocket and ask us not to disclose that service to a health plan for payment or operations, we will comply unless disclosure is required by law.
  • Receive a list of certain disclosures we made of your health information during the period permitted by law.
  • Receive a paper copy of this notice at any time, even if you previously agreed to receive it electronically.
  • Choose a personal representative, when legally authorized, to exercise your rights and make choices about your health information.
  • File a complaint if you believe your privacy rights were violated. We will not retaliate against you for filing a complaint.

Your choices

For certain health information, you may tell us your preferences about what we share. When applicable, you may direct us to share information with family members or others involved in your care, participate in disaster-relief communications, or contact you for fundraising. We do not sell your health information. We will obtain written authorization before using or disclosing protected health information for marketing when authorization is required, and for most uses or disclosures of psychotherapy notes. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it.

How we may use and disclose your information

We may use or disclose your health information as permitted or required by law to:

  • Provide, coordinate, or manage your treatment and communicate with other professionals involved in your care.
  • Operate our practice, improve services, supervise clinicians, conduct quality activities, and contact you about appointments or care.
  • Bill and obtain payment from health plans or other responsible parties.
  • Support public health and safety activities, including reporting suspected abuse, neglect, or domestic violence when required; preventing or reducing a serious and imminent threat; and reporting certain diseases or adverse events.
  • Comply with health oversight activities, audits, inspections, investigations, licensing requirements, and applicable federal or state law.
  • Respond to lawful requests related to workers’ compensation, law enforcement, courts, administrative proceedings, coroners, medical examiners, and organ or tissue donation.
  • Conduct research when legally permitted and appropriate safeguards are in place.

Other uses and disclosures not described in this notice will be made only with your written authorization unless otherwise permitted or required by law.

Our responsibilities

We are required by law to maintain the privacy and security of protected health information, provide this notice of our legal duties and privacy practices, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required by law. We will inform you if a material change is made to this notice.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.

Website Privacy and Electronic Communication

This public website is intended for general educational and administrative information. It is not a patient portal and should not be used to send diagnoses, symptoms, treatment details, Social Security numbers, insurance identification numbers, or other sensitive health information.

Standard email and ordinary website communications may not be secure. Use the secure client portal or another practice-approved method for clinical records, forms, treatment communication, and protected health information. Email is not monitored continuously and should not be used for emergencies.

The website may collect limited technical information, such as browser type, device information, pages visited, and cookie preferences, to operate and improve the website. B & M Health Services does not sell personal information. Third-party services and links have their own privacy practices. Do not enter protected health information into non-secure website fields.

Good Faith Estimate and No Surprises Information

If you do not have health insurance, or you choose not to use insurance to pay for care, you generally have the right to receive a Good Faith Estimate explaining the expected cost of scheduled services.

  • You may request a Good Faith Estimate before scheduling or at any time.
  • If services are scheduled at least three business days in advance, an estimate will generally be provided within the timeframe required by federal law.
  • The estimate is not a contract and actual charges may differ if your needs or treatment plan change.
  • If a final bill is at least $400 more than the Good Faith Estimate from a provider or facility, you may be eligible to use the federal patient-provider dispute-resolution process.
  • Keep a copy of your estimate and billing records.

Insurance benefits are determined by the health plan and are not a guarantee of payment. Clients remain responsible for understanding deductibles, copayments, coinsurance, authorization requirements, non-covered services, and out-of-network obligations.

Emergency and Crisis Information

B & M Health Services is an outpatient practice and does not provide emergency services through this website, email, voicemail, or the consultation request system.

  • If you or another person is in immediate danger, call 911 or go to the nearest emergency department.
  • For suicidal thoughts, emotional crisis, or urgent support in the United States, call or text 988, or visit 988lifeline.org.
  • Do not use email or website messages for emergencies. These channels are not monitored continuously.

Website Information Is Not Medical Advice

Website information is educational and does not establish a therapist-client relationship. A professional relationship begins only after required intake procedures are completed and services are formally accepted. Website content is not a diagnosis, treatment recommendation, or substitute for evaluation by a qualified health professional.
