How to Submit a Referral

To protect client privacy, B & M Health Services does not collect referral or clinical information through this website. Select “Submit a Secure Referral” below and enter all client and referral information only through our secure SimplePractice system.

Do not send diagnoses, symptoms, treatment records, insurance identification numbers, or other protected health information through ordinary email or website messages. A referral does not authorize the exchange of treatment records. A separate valid authorization may be required for coordination of care.

Information You May Be Asked to Provide Securely

Client contact and demographic information; insurance and access information; requested service; reason for referral; urgency and safety considerations; referring-provider name, specialty, contact information, and NPI when applicable; whether the client or guardian is aware of the referral; and coordination-of-care authorization status.
